The Step-by-Step Guide to Completing Your Discovery Health Application

1. Getting Started.

There are 2 ways to apply:

Complete this request and Discovery will contact you to complete online.

This is the fastest way for you to become a Discovery health member!

2. Personal Information.
Fill in your personal details accurately.
Think of it as introducing yourself to Discovery Health.

3. The Plan Options
Look at the various health plans offered by Discovery Health.
Which one suits your lifestyle and needs?

Investigate the various plans Discovery offer, and choose a plan that best suits your needs.

4. Confirm the premium you will pay
What the various plans costs, and how do they fit with your budget?

5. Adding Dependants
If you have dependants, include them in your application by inserting their details..

6. Medical History - Honesty is the Best Policy
Disclose your medical history up-front, and you will avoid having your medical aid terminated through non-disclosure in the future.

7. Submitting Documents
Submit IDs or birth certificates of all members.
If you are joining KeyCare, submit a current bank statement.
Submit certificates of membership from all past medical aids.
If you cannot obtain them, then an affidavit stating the medical aid/employer name and dates of membership will suffice.

We will tell you what you need to submit.

8. Payment
Medical aid is billed monthly in advance and Discovery Health will run a debit order on the bank account you provide.
Look at your health as an investment, and make sure it's well taken care of by always paying the monthly premium on time!

11. Accessing Member Benefits
Download the Discovery App and access your membership details, plan benefits and limits and much, much more!

Congratulations, you're now a Discovery Health member!
