1. Health4Me Day-to-day Benefits
The ideal, affordable cover for your employees’ everyday healthcare needs.
There are three levels of day-to-day benefits to choose from, ranging from the top Gold plan, an intermediary Silver plan , through to the reduced benefit Bronze plan.
Covers expenses such as:
- Unlimited GP consultations through a wide network of doctors (GPs.) Access to benefits at Momentum CareCross Network GPs.
- Specialist benefit with the Gold plan.
- Hello Doctor, where members and their families can talk to a doctor by phone, anytime, anywhere, in any language - for free.
This can certainly reduce the need to take time off from work for a visit to the doctor, as well as save the expense associated with a doctor consultation. - Over-the-counter medication.
- Flu vaccination.
- A comprehensive maternity benefit, offering antenatal GP visits, 2 2D foetal growth scans, antenatal vitamins and antenatal pathology tests.
- Unlimited Chronic illness cover, including pathology and radiology monitoring when related to the chronic condition.
26 Chronic conditions are covered. (Only on the Gold plan.) - HIV benefit. (Only on the Gold plan.)
- Basic and emergency dentistry at network dentists. Fillings, extractions, infection control, cleaning and polishing of teeth.
(Not with the Base plan) - Basic optometry benefits every 2-years. (Not with the Base plan)
- 1 Positive COVID-19 screening test is covered per member, per year.
- An annual health assessment paid.
- Advice for road accident and injury on duty claims.
2. Health4Me Major medical events benefits
Major medical events benefits (which can only be taken in conjunction with Health4Me Day-to-day Benefits.) provide your employees with access to accident and emergency cover, a hospital cash benefit, maternity lump sum benefit, and funeral benefit (including a repatriation benefit).
There are 2 levels of medical event benefits - the comprehensive Standard option and the limited Base option.
Both plans offer:
Accident and emergency cover which has 3-benefits:
- Accident cover consisting of a Casualty and an In-hospital benefit at a private hospital.
An accident means a medical emergency that is an external, unexpected event that is not traceable, directly or indirectly, to a member’s state of mental or physical health prior to the event. - Emergency (heart attack or stroke), that meet the qualifying criteria, are covered at a private hospital.
- Emergency transportation, stabilisation and treatment cost paid in case of an accident that requires immediate medical treatment.
Accident and emergency cover includes emergency transportation, stabilisation and treatment, diagnostic scans (like MRI and CT scans), take-home medication, internal and external prosthetics, orthotics and assistance devices, rehabilitation services (like step-down services, wound care, physiotherapy and occupational therapy), subject to both clinical approval and per event limits.
In the case of medical emergencies that do not fit the qualifying criteria, transportation, stabilisation and treatment at a State facility will be covered.
Hospital cash and maternity lump sum benefit.
- The hospital cash benefit pays you a daily benefit amount per day spent in hospital, provided that you are in hospital for more than two days. This benefit helps you to pay for any unexpected costs that you may have due to being hospitalised, such as transport for your family to visit you while you are in hospital, or to supplement your income due to time taken off from work.
- A Maternity lump sum benefit is paid if hospitalisation is due to childbirth, irrespective of the number of days that the member has been hospitalised.
Funeral
The funeral benefit will help pay for the funeral expenses in the event that the member, spouse or a child passes away.
It includes a repatriation benefit should the member or a listed dependent, pass away within South Africa or a neighbouring country, (Botswana, Lesotho, Mozambique, Namibia, Swaziland or Zimbabwe), more than 100kms from their normal place of residence.
