Can You Change Your Medical Aid Plan Within the Same Scheme?
South African medical aid members often want to upgrade or downgrade their plans.
Most schemes allow option changes during specific periods, generally taking effect in January, but rules differ between schemes and plan types.
Some schemes allow limited mid-year upgrades, while downgrades are strictly restricted to year-end.
It is therefore critical that you understand timelines, underwriting implications, and benefit changes before switching.
It is always better to try and remain with your current medical aid if possible, as there are waiting periods when joining a new scheme. We will help you with everything required to investigate, choose a new plan, and complete the switching process.
Here is how we can help:
- Some upgraded plan premiums are extremely high! Let’s see if there is a more balanced option for you.
- Check that you are not paying for benefits you do not need.
- Review and address any Late Joiner Penalty you may be subjected to.
However, to do this we need to be appointed as your medical aid broker so we can investigate your plan at no added cost to you. Even if you joined directly, you are still paying a broker fee as part of your premium!
So, use our extensive medical aid knowledge and experience — free of any charge.
If this is possible, complete and return this appointment form, and once appointed, we can help you.
Alternatively, please let me know if you prefer to look at other medical aids.
Remember, this appointment costs you nothing extra, but allows us to investigate, present options, and assist you if you decide to switch plans or schemes.
When Are You Allowed to Switch Medical Aid Plans?
Most schemes restrict internal plan changes to the medical aid annual option change period, typically October to December, with changes effective 1 January the following year.
- Annual option changes: Apply during the year-end window for the new benefit year.
- Mid-year upgrades: Fedhealth is one of the few schemes allowing mid-year upgrades for qualifying life-changing events (e.g. pregnancy or dread disease diagnosis).
- Downgrades: Generally restricted to the annual year-end option change window.
| Situation | Usually Allowed? | Typical Timing |
|---|---|---|
| Upgrade to higher plan | Yes | Annual window; Fedhealth allows mid-year for life events |
| Downgrade to cheaper plan | Usually yes | Annual option change window |
| Change hospital plan | Usually yes | Depends on scheme rules |
| Add savings option | Scheme dependent | Annual review period |
What Is Required to Change Your Medical Aid Plan?
- Completed option change form.
- ID verification.
- Updated income details where relevant for income-banded plans.
- Dependant updates.
- Debit order confirmations.
- Scheme approval.
We can help you manage all of this at no charge.
Will Waiting Periods Apply When You Change Plans?
According to the Medical Schemes Act, a medical scheme may not impose a general or condition-specific waiting period on a beneficiary changing from one benefit option to another within the same medical scheme, unless the beneficiary is still serving an unexpired waiting period from initial enrollment.
Internal option changes avoid new general waiting periods, though upgraded benefits may have specific waiting periods applied to new high-cost claims.
Late joiner penalties remain unchanged, and Prescribed Minimum Benefits (PMBs) remain fully protected.
Important: Always confirm exact rules with a licensed adviser like us.
What Happens to Your Benefits When You Switch Plans?
Switching options can affect hospital networks, day-to-day benefits, medical savings accounts, chronic medication formularies, co-payments, and specialist access.
Please contact us to make sure that you fully understand the implications for you when switching your plan.
Should You Upgrade or Downgrade Your Medical Aid?
You should consider upgrading your plan if you have upcoming specialist treatments, new chronic conditions, family planning needs, or require broader hospital cover.
Conversely, downgrading plans suits healthy members wanting to reduce monthly expenses (such as moving to a hospital plan and self-funding routine care) or those under budget pressure.
Always compare total clinical value and limits, not just the monthly premium.
Common Mistakes to Avoid When Switching Medical Aid Plans
- Choosing a new plan based purely on price.
- Ignoring hospital network restrictions.
- Missing annual option change deadlines.
- Failing to verify chronic medicine formulary inclusion.
- Downgrading immediately before scheduled procedures.
- Failing to calculate out-of-pocket exposure.
Private healthcare providers can charge up to 500% of medical scheme rates.
Medical aids add procedure co-payments as well.
These are significant costs you must consider.
A Gap or Top-Up plan is a separate insurance that covers most in-hospital claim shortfalls and any co-payments.
It is a vital safety net, ensuring you have enough to cover medical treatments, without having to pay for unexpected costs.
Joining a hospital plan with Gap Cover is the most affordable way for families to protect against rising costs in 2026.
Protect yourself by joining Zest's GAP/Top-up insurance plan.
Specialised Dental treatments can be extremely expensive!
The costs of braces, root canal, implants, and so on can run into thousands of rand.
Most treatments are done out-of-hospital and are paid from your savings.
That can result in less money available for other medical needs and family limits being used on one member only!

Frequently Asked Questions
Can I change my medical aid plan at any time?
Generally no. Most changes take place during the annual option change period, though certain life-event upgrades may be permitted.
Can I downgrade my medical aid mid-year?
Many schemes restrict downgrades to year-end. Always confirm how savings account allocations will be balanced.
Will I lose my waiting periods if I switch plans?
No new general waiting periods apply for internal scheme switches, though unexpired waiting periods carry over.
Does changing plans affect chronic medication cover?
Chronic cover continues, but formularies and benefit limits differ between plan tiers.
What happens to my medical savings account?
Balances generally transfer subject to scheme rules. However, if you have overspent your monthly savings allocation and switch to a lower plan, you must repay the overdrawn balance.
